About the role
The Therapeutic Act Worker provides direct services to clients in a mental health setting, supporting daily living activities and coordinating discharge planning. This position works within the Transitional Living Center of Lehigh Valley Health Network, a nationally recognized health system.
Key responsibilities
- Develop and implement individual treatment plans and client‑specific discharge plans.
- Provide instruction in activities of daily living in compliance with program, county, and state regulations.
- Report maintenance issues, maintain program facilities and client apartments, and participate in performance‑improvement activities.
- Coordinate financial resources for clients and assist the program nurse with medication observation and documentation.
Required profile
- High School Diploma or GED with at least 4 years of experience in a mental health setting.
- Bachelor’s Degree in Human Services preferred.
- Ability to lift and carry 40‑50 lb, stand or walk continuously, and perform patient transport duties.
