RN Case Manager – PRN – Mon‑Fri 8am‑4:30pm

About the role

Ochsner is seeking a Registered Nurse Case Manager on a PRN basis to support patient care during weekdays. The role involves coordinating multidisciplinary communication, reviewing hospitalizations, and ensuring appropriate utilization of resources while respecting patient self‑determination.

Key responsibilities

• Perform pre‑admission, admission, and ongoing stay reviews according to CMS and payer guidelines.
• Collaborate with physicians, caregivers, and payers to develop and implement individualized discharge plans.
• Serve as liaison among emergency department, hospitalists, attending physicians, and case management teams.
• Maintain documentation of level‑of‑care orders and ensure compliance with regulatory standards.

Required profile

• Current RN license in the state of practice and Basic Life Support (BLS) certification.
• Preferred: Bachelor’s degree in nursing and 3 years of hospital‑based discharge planning, case management, or utilization review experience.
• Ability to work a flexible schedule, including weekdays from 8 am to 4:30 pm.

Required skills

• Proficiency with computers, software, and web‑based applications.
• Basic Life Support (BLS) certification.
• Case Management Certification (CCM) or Accredited Case Manager (ACM) preferred.
