About the role

The RN Case Manager provides multidisciplinary communication and facilitates patient care throughout hospitalization, ensuring appropriate resource utilization while respecting patient self‑determination.

Key responsibilities

  • Review admissions, ongoing stays, and discharge plans according to CMS and payer guidelines.
  • Collaborate with ED physicians, hospitalists, and third‑party payers to determine appropriate level of care and alternative options.
  • Serve as liaison among patients, caregivers, care team members, and post‑acute providers.

Required profile

  • Graduate of an accredited nursing program; Bachelor’s degree preferred.
  • Current RN license in the state of practice and Basic Life Support certification.
  • Ability to work flexible schedules, including nights, weekends, holidays, and on‑call.

Required skills

  • Proficiency with computers, software, and web‑based applications.
  • Effective verbal and written communication for presenting information to diverse audiences.
  • Strong interpersonal skills for teamwork and patient interaction.
