About the role

We are looking for compassionate individuals to join our remote team as Social Care Screeners. In this role you will support a Medicaid 1115‑Waiver Social Care Network in New York, helping members access essential health and social services.

Key responsibilities

Conduct standardized social care screenings with members.
Identify health‑related social needs and accurately document findings.
Maintain confidentiality and comply with HIPAA and program requirements.
Communicate professionally and empathetically with diverse individuals.
Ensure timely and accurate completion of assigned screenings.

Required profile

Minimum 1 year of experience in social care, case management, patient navigation, or related screening work.
Strong communication and interpersonal abilities.
Excellent organizational skills with attention to detail.
Ability to work independently while meeting productivity and quality expectations.
Bilingual or multilingual candidates (English plus Spanish, Haitian Creole, or other New York languages) are strongly encouraged.
