About the role

The RN Navigator provides liaison and communication between patients, caregivers, providers, and the multidisciplinary team in the Heart Failure Clinic, ensuring smooth transitions and continuity of care.

Key responsibilities

Develop relationships with patients and their care teams to navigate treatment and follow‑up.
Coordinate discharge planning, post‑discharge care, and readmission reduction for high‑risk patients.
Collaborate with physicians and multidisciplinary staff to maintain up‑to‑date, patient‑centered care plans.
Identify barriers to care and suggest process improvements.

Required profile

Registered Nurse diploma; Associate’s or Bachelor’s degree preferred.
Minimum 2 years of nursing experience.
Current RN license in the state of practice and Basic Life Support (BLS) certification.
Ability to perform physical tasks such as lifting up to 20 lb regularly and standing for prolonged periods.

Required skills

Proficiency with Windows‑style applications and data entry.
Effective verbal and written communication.
Basic Life Support (BLS) certification.
