Registered Nurse – Patient Navigator

About the role

The Registered Nurse – Patient Navigator serves as the primary point of contact for patients, families, and referring physicians, helping to coordinate care and reduce fragmentation. This role supports both new and existing patients, ensuring smooth navigation through treatment plans and promoting quality improvement.

Key responsibilities

• Assist patients and families in navigating medical care, finding solutions to treatment‑related problems.
• Assess and monitor continuity of care, expedite service delivery, and track patient progress.
• Collaborate with the medical team for follow‑up and monitor outcomes.
• Participate in developing, implementing, and reviewing quality metrics and patient education materials.
• Compile data, track outcomes, and recommend process improvements.
• Provide referrals and support for uninsured or underinsured patients.

Required profile

• Specialized nursing diploma with at least 2 years of RN experience.
• Ability to maintain confidentiality of records and information.
• Knowledge of patient education techniques, community resources, and obesity management practices.
• RN license (PA – State of Pennsylvania) or equivalent.

Required skills

• Proficiency in Microsoft Office applications.
• Skilled in medical and psycho‑social assessment.
