About the role

The Lehigh Valley Health Network is seeking a Registered Nurse to serve as a Hospice Case Manager at Abington Hospital. This full‑time, onsite position offers a $10,000 sign‑on bonus and the opportunity to coordinate home‑based hospice care for a diverse patient caseload.

Key responsibilities

• Coordinate and direct delivery of care for patients receiving hospice services at home, collaborating with an interdisciplinary team.
• Develop, review, and revise individualized plans of care in partnership with physicians and the care team to meet patient goals and quality metrics.
• Perform initial and ongoing assessments, skilled treatments, and provide education to patients and caregivers on end‑of‑life issues, pain management, and discharge planning.
• Collect and report CMS‑required data accurately to support quality outcomes and reimbursement.

Required profile

• Graduated from an NLN‑approved nursing program with a current RN license in Pennsylvania.
• At least one year of recent direct patient care experience, preferably in home health or hospice.
• Ability to manage a caseload independently, travel to patient homes, and communicate complex care goals effectively.
• Strong organizational, time‑management, and documentation skills.

Required skills

• Basic Life Support (BLS) certification – American Heart Association.
• Valid Pennsylvania driver’s license.
• Proficiency with multiple computer applications and electronic medical record (EMR) platforms.
