About the role
The Lehigh Valley Health Network is seeking a full‑time Registered Nurse to serve as a Hospice Case Manager for patients in the Easton/Bethlehem territory. You will coordinate home‑based hospice care, collaborate with an interdisciplinary team, and ensure high‑quality, patient‑centered outcomes.
Key responsibilities
- Coordinate and direct care for an assigned caseload of home‑based hospice patients in partnership with physicians and the interdisciplinary team.
- Develop, review, evaluate, and revise individualized plans of care to meet patient goals, quality metrics, and level‑of‑care transitions.
- Perform initial and ongoing assessments, skilled treatments, and patient/caregiver education on end‑of‑life and palliative topics.
- Collect and submit CMS‑required data accurately to support quality outcomes and reimbursement.
- Communicate significant changes in patient status promptly to the care team.
Required profile
- Graduated from an NLN‑approved nursing program with a current RN license in Pennsylvania.
- At least one year of recent direct patient care experience, preferably in home health or hospice.
- Ability to manage a caseload independently, travel to patient homes, and work without direct supervision.
- Strong time‑management, organizational, and communication skills.
- Valid driver’s license for Pennsylvania (or New Jersey) and ability to lift/transfer patients as described.
Required skills
- Basic Life Support (BLS) certification.
- Proficiency with multiple computer applications and EMR platforms.
- Experience completing regulatory data sets with high accuracy.
- Knowledge of Conditions of Participation and hospice/palliative care standards.
- Board‑certified Ambulatory Care Nursing (or pursuit within 3 years) and/or Certified Hospice and Palliative Nurse (CHPN) credential.
