Physical Therapist – Home Care

About the role

Lehigh Valley Health Network is seeking a licensed Physical Therapist to provide home‑care services. The role focuses on restoring function, preventing disability, and helping patients achieve maximum independence through evidence‑based therapy.

Key responsibilities

• Develop and implement individualized physical therapy plans of care, including therapeutic exercises, manual techniques, functional mobility training, ADL training, and modalities.
• Perform patient assessments, reassessments, and modify treatment plans as needed.
• Document all care according to regulatory and departmental standards.
• Communicate patient progress and limitations to supervisors, physicians, and the care team.

Required profile

• Bachelor’s Degree in Physical Therapy (Master’s or Doctorate preferred).
• Licensed PT in Pennsylvania (required upon hire).
• Basic Life Support certification (American Heart Association) required upon hire.
• Ability to travel to patient homes and lift/carry up to 50 lb.
