Outpatient Clinic Nurse (RN)

About the role

The Outpatient Clinic Nurse provides on‑site clinical support for multiple Pulmonary and Sleep clinics, handling patient triage, medication management, RN visits, and care coordination. The role works closely with the RN team and clinic staff to ensure exceptional patient care and a collaborative environment.

Key responsibilities

• Triaging patient calls, escalating to APP team or MD as needed.
• Scheduling audio visits and follow‑up appointments for priority patients.
• Assisting with the Lung Health Initiative program coordination.
• Conducting nurse visits, patient education on medications and disease states.
• Managing medications, including prior authorizations and monitoring.
• Coordinating care for Endobronchial Valve candidates with physicians and care team.

Required profile

• RN degree required; BSN preferred.
• 1–3 years of nursing or healthcare experience.
• Strong clinical knowledge and ability to assess and triage patients both over the phone and in person.
• Excellent communication, organizational, and time‑management skills.
• Ability to lift up to 30 lbs, sit for extended periods, and drive between clinic locations.

Required skills

• Proficiency with EMR systems.
• Computer literacy and basic technical troubleshooting.
