On Call Registered Nurse (RN) Home Care – $20,000 Sign‑On Bonus

About the role

The Lehigh Valley Health Network is seeking a full‑time On‑Call Registered Nurse for Home Care. This position provides clinical support and coordination of patient care during and after normal business hours, with a $20,000 sign‑on bonus for qualified candidates.

Key responsibilities

• Perform telephone triage for patients calling after hours, using critical thinking and sound clinical judgement.
• Conduct patient assessments, deliver direct nursing care, and address physical, psychosocial, and environmental needs.
• Enter data, review clinical records, and complete other documentation tasks related to patient care.
• Provide education on end‑of‑life issues, palliative care, pain management, home care, hospice, and discharge planning.
• Communicate hand‑off information to the next shift RN and interdisciplinary team members.

Required profile

• Graduate of an NLN‑approved nursing program with at least 1 year of recent direct care experience.
• Current Pennsylvania RN license (or obtain within 30 days of hire) and BLS certification.
• Strong critical thinking, problem‑solving, and communication skills.
• Ability to multitask, work independently, and manage interruptions.
• Valid driver’s license and reliable automobile with insurance meeting PA or NJ minimum requirements.
