Occupational Therapist

About the role

The Occupational Therapist will manage a caseload of young adult clients in the Confluence Transitions program, helping them develop daily living, executive functioning, and independence skills. This role is part of a dynamic, collaborative team focused on mental health and community support.

Key responsibilities

• Conduct weekly individual and group OT sessions, including assessments, treatment planning, and discharge planning.
• Collaborate with mentors, coaches, and the clinical team to support participants and enhance the therapeutic process.
• Evaluate medical history, observe daily tasks, and test physical, mental, and cognitive abilities.
• Design personalized treatment plans with therapeutic activities and exercises.
• Teach daily living skills such as dressing, eating, bathing, and cooking.
• Recommend home, school, or workplace adaptations and assistive equipment.
• Educate families and caregivers on safe support strategies.
• Track and document client progress, adjusting plans as needed.
• Participate in weekly clinical meetings and on‑call emergency support.
