Therapeutic Case Manager

About the role

The Therapeutic Case Manager at Arete Transitions works directly with young adults to support their transition to independent living. Reporting to the Executive Director, the role blends direct participant support, program development, and administrative coordination.

Key responsibilities

• Lead two weekly coaching sessions and a weekly family check‑in for each participant.
• Coordinate with Life Skills Instructors, therapists, counselors, and external providers to align participant goals.
• Assess and resolve program challenges, manage schedules, budgets, and participant logistics.
• Develop and deliver curriculum based on the 9 Pillars of Whole Person Health model.
• Prepare coaching reports, family check‑in reports, and benchmark review materials.
• Support participant budgeting, housing care, and community activity planning.

Required profile

• Master’s level training in Human Services (e.g., MSW, LICSW).
• At least 3 years of experience in residential treatment, social services, or related fields.
• Inclusive, collaborative, organized, and able to manage multiple tasks independently.
• Strong problem‑solving mindset with a focus on continuous service improvement.
