Medical Director

About the role

Habitat Health is seeking a senior clinical leader to serve as the Medical Director for a PACE center in Chicago. The role balances direct patient care with oversight of clinical quality, safety, participant experience, and fiscal stewardship.

Key responsibilities

• Provide evidence‑based, person‑centered primary care to approximately 50% of the clinical panel, acting as supervising physician.
• Collaborate with the interdisciplinary team (IDT) on care planning across medical, behavioral, and functional domains.
• Lead safe medication management, including deprescribing and reduction of polypharmacy.
• Oversee care delivery across clinic, home, inpatient, and skilled‑nursing‑facility settings.
• Identify early functional or cognitive decline, conduct goals‑of‑care conversations, and use complex cases for IDT teaching.
• Partner with the Executive Director as a dyad leader to drive center performance, operational reliability, and fiscal responsibility.

Required profile

• Licensed physician able to provide direct clinical care and supervise other clinicians.
• Experience working with older adult populations and interdisciplinary teams.
• Strong clinical judgment in evaluation, diagnosis, and management of geriatric patients.
• Commitment to person‑centered care, quality improvement, and safety.
