Inpatient Discharge RN Navigator – Oncology & BMT (Full Time)

About the role

The Oncology & BMT Discharge Nurse Navigator serves as a clinical expert, patient advocate, educator, and care coordinator for hospitalized oncology and blood‑and‑marrow transplant patients transitioning to the next phase of care. The role ensures safe, seamless discharge, continuity of care, and timely access to outpatient services.

Key responsibilities

• Develop therapeutic relationships, conduct nursing assessments, identify discharge barriers, and provide individualized education on medications, symptom management, and follow‑up care.
• Coordinate multidisciplinary discharge planning with physicians, nursing staff, pharmacy, social work, case management, rehabilitation, home health, infusion centers, transplant teams, and community resources.
• Schedule follow‑up appointments, specialty consultations, labs, imaging, and infusion visits; facilitate referrals to supportive services such as nutrition, psycho‑oncology, palliative care, and financial counseling.
• Maintain accurate documentation in the electronic medical record and participate in quality‑improvement initiatives aimed at reducing readmissions.

Required profile

• Graduate of an accredited nursing program; RN license in the state of practice.
• Minimum 2 years of nursing experience; oncology, hematology/BMT, or care‑coordination experience preferred.
• Current Basic Life Support (BLS) certification.
• Ability to work a flexible schedule and travel between facilities as needed.

Required skills

• Proficiency with computers, software, and web‑based applications.
