Inpatient Care Manager – Registered Nurse

About the role

The Inpatient Care Manager – Registered Nurse coordinates comprehensive care for a designated patient population across the care continuum. This position blends clinical nursing, care management, and education to ensure safe, timely discharge and high‑quality outcomes.

Key responsibilities

• Conduct comprehensive admission assessments, develop individualized care and transition plans, and coordinate safe discharge including post‑acute authorizations, transportation, and home‑care services.
• Monitor patient progress through daily reviews and multidisciplinary rounds, identify and resolve barriers to discharge, and document all actions in the electronic medical record.
• Advocate for patients and families, address clinical, educational, and psychosocial needs, and facilitate communication among physicians, team members, and external agencies.

Required profile

• Bachelor’s degree in Nursing with at least 3 years of experience in care coordination, disease management, home health, hospital, or physician office settings.
• Current RN license in the state of practice (PA or NJ upon hire) and ability to work day shift with occasional weekend, holiday, and on‑call rotations.
• Strong knowledge of Medicare, Medicaid, and other payer systems, including benefits and reimbursement methodologies.

Required skills

• Proficiency with electronic medical record systems, specifically EPIC.
• Case management expertise, with accreditation (ACM) or certification (CCM) expected within 3 years.
• Ability to prioritize and coordinate multiple care plans efficiently.
