Home Infusion Pharmacist

About the role

The Home Infusion Pharmacist provides comprehensive medication management for patients receiving infusion therapy at home. Working within a multidisciplinary team, you will ensure safe, rational, and cost‑effective drug therapy while delivering patient education and clinical pharmacy services.

Key responsibilities

• Evaluate physician orders, considering allergies, renal function, lab values, dosing guidelines, and patient condition.
• Prepare, dispense, and verify sterile and investigational medications, maintaining QA documentation.
• Manage drug therapy to optimize outcomes through interventions, adverse‑drug‑reaction assessment, and patient education.
• Maintain licensure, certifications, and continuing education requirements.
• Ensure pharmacy operations comply with state and federal regulations.

Required profile

• Bachelor of Science in Pharmacy (Pharm.D preferred).
• Current pharmacy license in the state of practice.
• Effective verbal and written communication skills.
• Ability to work a flexible schedule, including on‑call availability.

Required skills

• Proficiency with Windows‑style applications and data‑entry software.
• Computer dexterity for retrieving patient information.
• Skills in compounding pharmaceuticals and operating technical equipment.
