Home Care Triage Nurse – Bucks, Montgomery & Chester Counties

About the role

The Home Care Triage Nurse provides clinical support to LVHN’s Home Care and Hospice services across Bucks, Montgomery and Chester counties. Working full‑time day shifts, you will triage patients by phone, coordinate care, and ensure accurate data entry and scheduling.

Key responsibilities

• Conduct phone triage using critical thinking and sound clinical judgment.
• Enter orders, schedule patients and staff, and perform other clinical documentation tasks.
• Communicate hand‑offs and status updates to the care team, flagging patients who need follow‑up visits or calls.

Required profile

• Graduated from an NLN‑approved nursing program with at least one year of nursing experience in a healthcare setting.
• Demonstrated critical thinking, analytical, and problem‑solving abilities.
• Excellent verbal and written communication, customer service, and time‑management skills.
• Ability to multitask, adapt to changing priorities, and work both independently and as part of a team.

Required skills

• Basic Life Support (BLS) certification – American Heart Association.
• Licensed Registered Nurse (RN) in Pennsylvania.
• Basic computer proficiency for data entry and scheduling.
• Valid driver’s license for Pennsylvania (or New Jersey as required).
