About the role
Advantmed is seeking a licensed Nurse Practitioner to perform structured, in‑home wellness and risk‑adjustment assessments for Medicare members and other at‑risk populations in Warren County, OH. This PRN, 1099 position offers flexible weekday scheduling, paid mileage, and a reliable stream of visits with full operational support.
Key responsibilities
- Conduct in‑home Annual Wellness Visits and chronic condition assessments.
- Perform focused histories, physical exams, vitals, medication reviews, and required screenings (e.g., Hemoglobin A1C, Spirometry, Kidney Health Evaluation, Diabetic Retinal Exam, FIT).
- Educate members on preventive care and chronic disease management.
- Document all visits in the EMR to support value‑based care and risk adjustment.
Required profile
- Active, unencumbered Nurse Practitioner license in Ohio (or ability to obtain additional licensure).
- Minimum 3 years of patient‑care experience preferred.
- Reliability and ability to maintain ~30 hours of monthly availability.
- Willingness to travel within a 55‑mile radius of the assigned county.
Required skills
- Proficiency with Electronic Medical Record (EMR) systems.
