Field Based HSS Coordinator - Hidalgo County, TX

About the role

UnitedHealthcare is hiring a Field Based HSS Coordinator for Hidalgo County, TX and surrounding areas. The role is part of an Integrated Care Model: it relays information about a member's needs, advocates for the best possible care and ensures members get the right services. It is a field-based position with a home-based office; candidates living within 100 miles of Hidalgo County can work on an office-based/field schedule.

Key responsibilities

• Complete face-to-face assessments in members' homes to address unmet needs
• Travel up to 75% of the time, including up to 100 miles from the work location, to meet business needs
• Meet documentation and turnaround times based on state contractual requirements
• Coordinate members' services and work on service plans
• Create, edit, save and send documents, spreadsheets and emails

Required profile

• One of: high school diploma or GED with 5+ years of experience with the ABD/SSI population in a clinical healthcare role; associate's degree with 3+ years of clinical experience; unrestricted LPN/LVN license in Texas with 3+ years of clinical experience; CHW certification with 3+ years of clinical experience
• Commutable distance from assigned duties and access to reliable transportation
• Valid US driver's license and ability to travel 75% of the time in the assigned area to visit Medicaid members in their homes, community centers, hospitals, nursing facilities or provider offices
• High-speed internet at home

Required skills

• Intermediate proficiency with Microsoft Office (Word, Excel, Outlook)
• Time management and organizational skills
