Community Case Expeditor – Behavioral Health (Augusta)

About the role

The State of Georgia’s Department of Behavioral Health and Developmental Disabilities is seeking a Community Case Expeditor in Augusta. Reporting to the Regional Services Administrator, you will coordinate and facilitate timely access to community‑based services for individuals transitioning from hospitals or other institutional settings.

Key responsibilities

• Collaborate with hospitals, providers, families, courts and law‑enforcement to plan and execute discharge and transition processes.
• Provide customer‑service functions such as explaining state and regional services, handling inquiries and complaints.
• Facilitate access to recommended services, linking individuals with appropriate community resources before, during and after discharge.
• Monitor and track transitions, identify barriers, and develop solutions with providers to ensure continuity of care.
• Offer consultation, technical assistance and education to providers, hospitals and community partners.
• Support training and educational activities related to transition planning and community‑based services.

• Ability to work under limited supervision of the Regional Services Administrator.
• Strong collaboration and communication skills with diverse stakeholders.
• Commitment to person‑centered planning and community integration.
