Clinical Pharmacist – Specialty Pharmacy

About the role

Clearway Health is seeking a Clinical Pharmacist to ensure optimal patient outcomes for chronic and specialty drug therapies. You will serve as a medication expert for patients, families, and healthcare teams while supporting specialty pharmacy operations.

Key responsibilities

• Conduct comprehensive medication therapy reviews for patients on specialty medications, ensuring safety, efficacy, and adherence.
• Partner with prescribers, nurses, case managers, and payors to facilitate initiation and continuity of specialty therapies, including prior authorizations and appeals.
• Monitor clinical outcomes, adverse events, and adherence metrics, documenting interventions for quality improvement.
• Maintain compliance with URAC/ACHC standards, manufacturer agreements, and payor requirements, supporting audits and accreditation.

Required profile

• PharmD or bachelor's degree from an accredited College of Pharmacy.
• PGY‑1 residency or 3+ years as a clinical pharmacist in direct patient care.
• Active pharmacist licensure in the state of employment (or ability to obtain at hire).
• Demonstrated expertise in medications, drug interactions, disease states, and therapeutic management.

Required skills

• Proficiency in Microsoft Office (Word, Excel, Outlook) and web browsers.
• Strong documentation and reporting abilities.

• Annual base salary range $146,000 – $182,000.
