Case Manager – Housing Services

About the role

The Housing Case Management Services (HCMS) Case Manager provides intensive, long‑term, housing‑focused case management to households experiencing prolonged homelessness and significant barriers to housing stability. The role supports participants in securing permanent housing and promotes retention, stability, and overall well‑being.

Key responsibilities

• Provide intensive case management to households experiencing prolonged homelessness, developing individualized housing and service plans.
• Assist participants with housing search, applications, landlord communication, and move‑in coordination.
• Connect participants to healthcare, behavioral health, benefits, employment, transportation, and other community resources.
• Maintain accurate documentation in HMIS and other required systems, and collaborate with internal teams, landlords, and community partners.

Required profile

• High school diploma, GED, or equivalent experience.
• Experience working with individuals or households facing homelessness or housing instability.
• Strong communication, organization, and interpersonal skills.
• Valid driver’s license, proof of automobile insurance, and reliable vehicle for site visits.

Required skills

• Proficiency with Microsoft Office.
• Proficiency with Google Workspace.
• Experience with cloud‑based client databases; HMIS experience preferred.
