Care Manager I

About the role

IOA provides case management services for older adults and adults with disabilities aged 18 and older, assessing their multiple medical and psychosocial needs and planning and monitoring quality care. The position is based in Merced.

Key responsibilities

• Conduct comprehensive and ongoing assessments of clients' physical, mental health, environmental and spiritual needs, and write them up.
• Develop and initiate a client-centered Community Living Plan and meet clients at least monthly, reviewing and modifying the care plan as needed.
• Make home visits, acute hospital and skilled nursing facility visits, and escort clients to medical appointments.
• Arrange and monitor community services, with knowledge of Medicare, Medi-Cal and other entitlement programs.
• Provide crisis intervention, advocacy, problem solving and therapeutic interventions, and educate clients and families about resources.
• Maintain open communication with community providers, monitor the quality of their services, and coordinate placement or termination of clients as appropriate.
• Document case management activity in progress notes within 24-48 hours, keep required paperwork, and take part in data collection, quality improvement and team meetings.
• Follow the ethical and legal standards of the National Association of Social Workers and the policies of the Community Living Fund and the Institute on Aging, and attend continuing education.

Required profile

• B.A. or B.S. in Social Work or a related field.
• One year of work with disabled adults and/or older adults.
• Understanding of the medical and psychosocial problems of functionally impaired adults and older adults.
• Experience with mental and/or behavioral health diagnoses and substance abuse disorders highly desired.
• Case management experience in the community health care delivery system.
• Detail-oriented, good problem solving and prioritization skills.
• Bilingual English/Spanish preferred.

Compensation range: $75,000 to $78,000 per year.
