Nurse Liaison

About the role

IOA, a provider of programs that help aging adults and adults with disabilities stay in their own homes, is hiring a Nurse Liaison in Merced. The role provides eight hours of support per week, educating and training RCFEs in the network, acting as content expert for waived services programs and serving as primary liaison with the Health Plan team. It works closely with the Director, social workers and supervisors to make sure quality measures are completed.

Key responsibilities

• Advise Care Management staff on medical issues such as diabetes, medication, wound care and other acute or chronic illnesses
• Take part in multidisciplinary team meetings and care conferences, recommending a safe level of care
• Help people moving out of institutional settings find long-term supports for safe community living
• Review discharge paperwork and consult with inpatient, outpatient and primary care providers
• Train CalAIM staff on the client population and program, and complete assessments and documentation for waived services programs
• Liaise with Care Management staff so clinical documentation follows best-practice guidelines
• Analyze utilization patterns of high-utilizers and help implement health-promotion interventions
• Work with CalAIM leadership to improve program efficiency, monitor provider quality, and take part in continuous quality improvement
• Follow Institute on Aging regulations and attend continuing education

Required profile

• California Registered Nurse (R.N.) license
• At least two years working with disabled adults and/or older adults as a registered nurse
• Experience training and educating a diverse group
• Preferred: experience with mental and/or behavioral health and substance abuse, and with a health plan or SNF/hospital setting
• Detail-oriented, good problem-solving and prioritization skills

Pay range: $53 to $56 per hour, depending on experience and education.
