Care Manager I - Community Living Fund

About the role

The Care Manager I provides case management for Community Living Fund clients, older adults and adults with disabilities aged 18 and older. The role assesses clients with multiple medical and psychosocial needs and plans and monitors services and interventions to ensure quality care.

Key responsibilities

• Conduct comprehensive and ongoing assessments of clients' physical, mental health, environmental and spiritual needs, and write them up
• Develop and initiate a client-centered Community Living Plan based on the assessment
• Carry out home visits, acute hospital and skilled nursing facility visits, and escort clients to medical appointments
• Arrange and monitor community services, with knowledge of Medicare, Medi-Cal and other entitlement programs
• Meet clients at least monthly, review and modify their care plan, and document case management activity in progress notes within 24-48 hours
• Provide crisis intervention, advocacy, problem solving and therapeutic interventions, and educate clients and families about resources
• Maintain open communication with community providers while keeping client information confidential, and evaluate whether services are provided in a timely manner
• Evaluate clients' ability to stay safely at home, coordinate placement when needed, and terminate clients when appropriate
• Take part in data collection, continuous quality improvement, team meetings, supervision and continuing education

Required profile

• B.A. or B.S. in Social Work or a related field
• One year of work with disabled adults and/or older adults
• Understanding of the medical and psychosocial problems of functionally impaired adults and older adults
• Experience with mental and/or behavioral health diagnoses and substance abuse disorders highly desired
• Case management experience in the community health care delivery system
• Detailed, problem-solving, able to prioritize multiple tasks

Compensation range: $71,000 to $78,000 per year.
