About the role

The Care Coordinator Associate supports the Care Coordination Manager in matching caregivers with clients, ensuring timely and appropriate care delivery. This full‑time, salaried position offers a fun team environment and benefits.

Key responsibilities

• Master the Wellsky software to manage client and caregiver information.
• Communicate with clients and caregivers via phone, email, and text to fill shifts and collect feedback.
• Resolve last‑minute caregiver absences by quickly finding qualified replacements.
• Provide coaching and support to caregivers to improve performance.
• Collaborate with clinical and recruiting teams and handle occasional weekend or evening coverage.
