Care Continuum Partner – Medical Assistant

About the role

The Care Continuum Partner – Medical Assistant works regionally to support patients and practices, both in‑person and virtually. This role bridges clinical teams and patients, ensuring smooth workflow, care plan compliance, and effective virtual care coordination.

Key responsibilities

• Assist with pre‑visit planning, care gap closures, referrals and medication authorizations.
• Conduct patient outreach via phone, patient portal and other virtual platforms to support appointment needs and care plan adherence.
• Facilitate virtual rooming, concierge calls for telehealth, and provide clear preparation instructions for tests and procedures.
• Manage prior authorizations, referral appointments, and support intake for annual well‑visits.
• Serve as a liaison for patients and families on patient portal and medical record technology.

Required profile

• Technical School Diploma or Specialized Diploma as a Medical Assistant.
• Minimum 2 years of patient interaction experience and 3 years in a complex health‑system environment.
• High accuracy, strong customer service, and ability to handle multiple tasks.
• Effective written and verbal communication with strict confidentiality.

Required skills

• Electronic medical records (EMR) and scheduling software proficiency.
• Medical terminology knowledge.
• CCMA certification (Certified Clinical Medical Assistant) and NHA certification.
• Basic computer skills and familiarity with patient portal technology.
