Care Continuum Partner

About the role

The Care Continuum Partner acts as the primary liaison between patients, families, and the clinical team, coordinating care across outpatient, procedural, community, and transition settings to ensure an optimal patient experience.

Key responsibilities

• Collaborate with clinical teams for pre‑visit planning, referral management, scheduling, pre‑certification, and follow‑up communications.
• Coordinate complex appointments—including procedures, surgeries, diagnostic testing, labs, and new‑patient intake—across multiple resources.
• Assess patient and family needs, support care‑plan compliance, and conduct outreach via phone, patient portal, or other virtual platforms.
• Provide clear preparation instructions for tests and procedures and deliver a comprehensive care‑continuum plan.
• Connect patients and families with internal resources such as financial counseling, transportation, social work, nutrition services, and support groups.

Required profile

• High School Diploma/GED (or higher) with a commitment to continued education.
• At least 2 years of direct patient interaction and 3 years working in a complex health‑system environment.
• Strong attention to detail, confidentiality, and ability to handle multiple tasks in stressful situations.
• Excellent written and verbal communication skills.

Required skills

• Working knowledge of electronic medical records and scheduling software.
• Familiarity with medical terminology.
• Basic computer functionality (keyboard, mouse, patient portal navigation).
