Bilingual Case Manager (Spanish/English)

About the role

The Bilingual Case Manager provides counseling, crisis intervention, and comprehensive case management to individuals accessing the Self‑Help Center. This role supports participants in housing, mental health, substance use, medical, and benefits advocacy using harm‑reduction and self‑help approaches.

Key responsibilities

• Conduct intakes, assessments, and develop individualized service plans.
• Facilitate support groups and connect participants to substance‑abuse treatment, medical, and community mental‑health services.
• Maintain case documentation and meet City & County data collection requirements.

Required profile

• Bilingual Spanish/English proficiency.
• 3 + years experience with poverty, homelessness, mental health, or substance‑use populations, or BA/BS in Social Work, Psychology, Counseling plus 2 + years case‑management experience.
• Demonstrated ability to work with clients facing multiple barriers and to apply harm‑reduction principles.

• Ability to work ethically in a team, maintain confidentiality, and manage caseloads.
