Clinical Pharmacist – Inpatient Hospital Pharmacy

About the role

Join a dynamic inpatient pharmacy team delivering high‑impact patient care in a hospital setting. As a Clinical Pharmacist you will work a mix of day and evening shifts, providing direct clinical support to patients and interdisciplinary teams.

Key responsibilities

• Provide clinical pharmacy services in an inpatient hospital environment.
• Review and verify medication orders for appropriateness and safety.
• Collaborate with physicians, nurses, and other healthcare professionals on patient care plans.
• Monitor therapy effectiveness, including pharmacokinetic dosing adjustments.
• Participate in code responses and emergency situations as needed.
• Educate patients and staff on proper medication use and safety.
• Support formulary management and medication safety initiatives.

Required profile

• Doctor of Pharmacy (PharmD) or BS Pharmacy degree.
• Minimum of 2 years of pharmacy experience; hospital experience preferred.
• State pharmacy licensure required within 60 days of start date.
• BLS and ACLS certifications required.
• PALS certification required for pediatric practice.
