About the role

Join Optum Home & Community Care as a HouseCalls Nurse Practitioner, providing annual clinical assessments in patients' homes across Northwest GA. This role offers a balanced work‑life schedule with no nights, on‑call, weekends or holidays required.

Key responsibilities

Conduct comprehensive in‑home assessments, including medical history, medication reconciliation, vital signs, and physical exam.
Perform evidence‑based screenings and point‑of‑care testing as appropriate.
Identify diagnoses and communicate findings to the member’s primary care provider.
Educate members on disease processes, medication adherence, and self‑care.
