Full-Time HouseCalls Nurse Practitioner - Texas Statewide Traveler

About the role

Optum House & Community Care, part of UnitedHealth Group, is hiring a full-time HouseCalls Nurse Practitioner to travel across the entire state of Texas about 85% of the time. The role is non-prescribing and field-based, focusing on annual in-home health assessments for Medicare Advantage and other plan members, with flexible scheduling between 8am and 8pm, seven days a week, with no on-call duty, weekends or holidays.

Key responsibilities

• Conduct comprehensive in-home assessments: past medical history review, medication reconciliation, vital signs and physical exam
• Perform evidence-based screenings and point-of-care testing as appropriate
• Identify and document diagnoses for care management and treatment planning
• Communicate findings to members' primary care providers to close care gaps
• Recognize urgent or emergent situations and intervene appropriately
• Educate members on disease processes, medications and compliance
• Address social determinants of health and refer to support services as needed

Required profile

• Active ANCC or AANP certification in Family, Adult, Geriatric or Adult-Gerontology Primary Care
• Active, unrestricted Texas licensure (or ability to obtain it by the start date) without disciplinary actions or practice limitations
• 1+ year of clinical experience (family, geriatric or home health preferred)
• Active, unrestricted driver's license and reliable transportation to complete home visits
• Ability to navigate varied home environments and to sit, stand or kneel as needed
• If practicing in a state where NPs practice independently, approval from the licensing board is required; new hires must apply within 1 month of starting

Preferred qualifications

• Proficiency with electronic medical records and technology
• Ability to carry equipment up to 30 pounds and climb stairs
• Communication skills with geriatric or Medicare populations
• Fluency in a language other than English
