Telehealth Nurse

About the role

The Telehealth Nurse provides remote clinical support, patient monitoring, triage, education, and care coordination for home‑health patients through telehealth and Remote Patient Monitoring (RPM) programs. Acting as the primary clinical contact, the nurse identifies early health concerns, intervenes to prevent hospitalizations, and improves patient outcomes.

Key responsibilities

• Monitor daily biometric data (blood pressure, pulse oximetry, weight, glucose, heart rate, temperature) and respond to RPM alerts.
• Conduct scheduled and on‑demand telehealth video visits with patients and caregivers.
• Identify early signs of clinical deterioration and provide chronic disease management for CHF, COPD, diabetes, hypertension, and post‑hospital recovery.
• Perform remote nursing assessments, triage symptoms, and escalate urgent conditions to physicians, field clinicians, or emergency services.
• Educate patients and caregivers on equipment use, disease management, medication adherence, and when to seek care.
• Collaborate with home‑health nurses, therapists, physicians, and care‑coordination teams to ensure continuity of care.
• Document all encounters, assessments, and escalations in the EMR while complying with HIPAA, Medicare/Medicaid, and organizational policies.

Required profile

• Current RN or LVN license.
• Experience delivering care via telehealth or remote patient monitoring.
• Strong clinical assessment skills and ability to recognize early signs of deterioration.
• Familiarity with EMR documentation and healthcare regulations (HIPAA, Medicare/Medicaid).
