Physical Therapist – Jacksonville

About the role

The Physical Therapist provides comprehensive assessment, planning, evaluation, and implementation of therapeutic care for assigned patients. Working on‑site in Jacksonville, the role ensures compliance with the Physical Therapy Practice Act while delivering high‑quality, patient‑centered treatment.

Key responsibilities

• Gather pertinent patient data from charts and other caregivers, consulting physicians when needed.
• Independently assess all assigned patient types and recognize additional needs during evaluation.
• Interpret diagnostic results, set measurable and realistic goals, and regularly reassess patient progress.
• Identify appropriate equipment, discuss options with patients and families, and coordinate its use.
• Maintain documentation and adhere to departmental and regulatory policies.

Required profile

• Bachelor’s degree in Physical Therapy (Doctorate preferred).
• Current LPT license in the state of employment.
• Basic Life Support (BLS) certification.
• Ability to work 8 AM – 5 PM, Monday through Friday on a per‑diem/as‑needed basis.
