Wound Care RN Case Manager

About the role

Lehigh Valley Health Network is seeking a registered nurse to serve as a Wound Care Case Manager. The RN will act as a consultant, educator, and researcher in enterostomal therapy and wound care, supporting patients, families, and clinical staff across the Bartonsville Health Center.

Key responsibilities

• Assess, plan, and implement patient and family care, evaluating interventions and revising care plans as needed.
• Develop and deliver teaching tools and programs tailored to patient and family needs.
• Create and maintain a priority list of active protocols for participating physicians.
• Formulate hospital policies and procedures related to enterostomal therapy and wound care.
• Serve as a patient advocate throughout all phases of wound care services.
• Identify areas for clinical studies and lead investigations.

Required profile

• Bachelor’s Degree in Nursing.
• Minimum 3 years experience in Enterostomal Therapy and Medical/Surgical Nursing.
• Competency caring for neonate to geriatric patients.
• Effective communication skills with staff, physicians, patients, and families.
• Valid Pennsylvania driver’s license and ability to lift 25 lb.

Required skills

• American Heart Association Basic Life Support (BLS) certification.
• Wound Care Certified (WCC) – to be obtained within 90 days.
• Enterostomal Therapy expertise.
• Proficiency with computer applications.
