Wellness Nurse (LPN)

About the role

Monarch Communities, a senior living company, is hiring a Wellness Nurse (LPN) in Tuckahoe. Led by the community's Health and Wellness Director (RN), the LPN provides direct nursing care to residents with an emphasis on holistic wellness and helps supervise the day-to-day nursing activities of the Certified Nursing Assistants. The position is full-time and part-time (7a-3p and every other weekend).

Key responsibilities

• Monitor the health, safety and well-being of all residents.
• Assist in training and monitoring of medication administration.
• Supervise care staff in line with state regulations and community policies.
• Maintain clinical quality assurance according to federal, state and local standards.
• Conduct thorough resident assessments.

Required profile

• Graduation from an accredited school of nursing with a current LPN license.
• At least 2 years' experience in long-term care, assisted living, home health or a hospital setting preferred.
• Experience working with residents with Alzheimer's or other related dementias.
• Ability to handle multiple tasks and knowledge of federal and state regulations and nursing practices.
• Physical demands include standing, walking, lifting and carrying up to 50 lbs and supporting residents; proper body mechanics are required.

Required skills

• Proficiency in computer skills, including Microsoft Office (Windows, Outlook, Excel) and the ability to learn new applications.
