Weekend Registered Nurse – Home Health & Senior Living

About the role

BAYADA Home Health Care is seeking a Per Diem Weekend Registered Nurse to join its home health senior living team, providing skilled nursing and rehabilitative care to clients across multiple counties in Central Pennsylvania.

Key responsibilities

• Conduct comprehensive head‑to‑toe assessments and identify subtle changes in patient condition.
• Provide acute and post‑acute care including CHF, COPD, wound care, IV therapy, and post‑surgical care.
• Triage and manage urgent clinical situations independently in the home setting.
• Perform medication reconciliation, patient education, and safety monitoring.
• Document wound assessments, care plans, and point‑of‑care notes efficiently.

Required profile

• Current Pennsylvania RN license in good standing.
• Graduated from an accredited nursing program.
• At least one year of recent hands‑on experience with adult/geriatric patients in acute, emergency, home health, hospice, ICU, med‑surg, sub‑acute, or rehab settings.
• Ability to work independently, prioritize urgent visits, and escalate concerns when needed.
• Strong problem‑solving skills and cultural sensitivity.
