Weekend Registered Nurse – Friday to Sunday

About the role

Join St. Landry HomeCare, a member of LHC Group, as a Weekend Registered Nurse providing compassionate hospice care to patients in their homes from Friday through Sunday. You will be responsible for delivering the full plan of care, ensuring high‑quality, patient‑centered services.

Key responsibilities

• Complete the initial nursing evaluation for hospice eligibility within 48 hours of referral.
• Identify and reassess patients’ physical, psychosocial, and environmental needs at least every 15 days.
• Initiate, coordinate, and document the plan of care, maintaining accurate clinical notes.
• Record problems, set goals, implement interventions, and track patient/family response.
• Collaborate daily with patients, families, physicians, and the interdisciplinary team.

Required profile

• Current, unrestricted RN license in the state of practice.
• Valid driver’s license, vehicle insurance, and reliable transportation or access to public transit.
• Current CPR certification.
• Ability to work in any home environment regardless of client demographics.
