Treatment Coordinator – Outpatient Mental Health

About the role

Lead care, support progress, and help clients stay connected to treatment. Community Wellness Outpatient Mental Health Center is seeking a qualified Treatment Coordinator to support clients receiving outpatient mental health services. This role is ideal for a licensed behavioral health professional who is organized, client‑centered, and experienced in care coordination, treatment planning, and clinical documentation.

Key responsibilities

• Provide weekly clinical support to assigned clients, including assessments, treatment planning, and progress monitoring.
• Coordinate care with therapists, psychiatric providers, case managers, and community resources.
• Track referrals for services such as housing, substance‑use treatment, primary care, and other community supports.
• Maintain accurate documentation in the electronic medical record and ensure compliance with agency, payer, and regulatory standards.
• Serve as Treatment Team Lead, coaching team members and resolving barriers to service delivery.

Required profile

• Active Maryland license or certification (e.g., LCPC, LCSW, LCMFT, LCADC, APRN‑PMH, Licensed Psychologist, etc.).
• Experience in outpatient or community mental health settings, with strong assessment, treatment planning, and care‑coordination skills.

Required skills

• Proficiency with electronic medical records (EMR) and basic computer applications.
