Treatment Coordinator – Outpatient Mental Health

About the role

The Treatment Coordinator will lead care, support progress, and keep clients connected to outpatient mental health services. This full‑time position works in a hybrid or onsite setting for a community‑focused mental health center.

Key responsibilities

• Provide weekly clinical support, assist with assessments, treatment planning, and progress monitoring.
• Coordinate care with therapists, psychiatric providers, case managers, and community resources.
• Track referrals for housing, substance‑use treatment, primary care, and other services.
• Maintain accurate documentation in the electronic medical record and ensure compliance with agency, payer, and regulatory standards.
• Serve as Treatment Team Lead, coaching team members and resolving barriers to service delivery.

Required profile

• Active Maryland license or certification (e.g., LCPC, LCSW, LCMFT, LCADC, APRN‑PMH, Licensed Psychologist, etc.).
• Experience in outpatient or community mental health settings with strong assessment, treatment planning, and care‑coordination skills.

Required skills

• Proficiency with electronic medical record (EMR) systems.
• Competence in basic computer applications.
