Treatment Coordinator – Outpatient Mental Health

About the role

Community Wellness Outpatient Mental Health Center is seeking a licensed behavioral health professional to serve as a Treatment Coordinator. In this role you will lead care, support client progress, and ensure continuity of treatment for outpatient mental health clients in Baltimore.

Key responsibilities

• Provide weekly clinical support to assigned clients, including assessments, treatment planning, and progress monitoring.
• Coordinate care with therapists, psychiatric providers, case managers, and community resources such as housing and substance‑use treatment.
• Track referrals and ensure clients receive needed services like primary care and PRP.
• Maintain accurate documentation in the electronic medical record meeting agency, payer, and regulatory standards.
• Serve as Treatment Team Lead, coaching team members and resolving barriers to service delivery.

Required profile

• Active Maryland license or certification (e.g., LCPC, LCSW, LCMFT, LCADC, APRN‑PMH, Licensed Psychologist, etc.).
• Experience in outpatient or community mental health settings, with strong assessment, treatment planning, and care‑coordination skills.
