Treatment Coordinator – Outpatient Mental Health

About the role

Lead care, support progress, and help clients stay connected to treatment. The Community Wellness Outpatient Mental Health Center is seeking a licensed behavioral health professional to serve as a Treatment Coordinator for outpatient mental health services. This full‑time position can be performed in a hybrid or onsite setting.

Key responsibilities

• Provide weekly clinical support, assist with assessments, treatment planning, and progress monitoring for assigned clients.
• Coordinate care with therapists, psychiatric providers, case managers, and community resources, tracking referrals for housing, substance‑use treatment, primary care, and more.
• Maintain accurate documentation in the electronic medical record and ensure compliance with agency, payer, and regulatory standards.
• Serve as Treatment Team Lead, offering coaching, feedback, and resolving barriers to service delivery.

Required profile

• Active Maryland license or certification (e.g., LCPC, LCSW, LCMFT, LCADC, APRN‑PMH, Licensed Psychologist, etc.).
• Experience in outpatient, community, or behavioral health settings preferred.
• Strong leadership, communication, organization, and time‑management abilities.

Required skills

• Proficiency with electronic medical record (EMR) systems.
• Basic computer applications.
