About the role

Lead care, support progress, and help clients stay connected to treatment. Community Wellness Outpatient Mental Health Center is seeking a qualified Treatment Coordinator to support clients receiving outpatient mental health services. This role is ideal for a licensed behavioral health professional who is organized, client‑centered, and experienced in care coordination, treatment planning, and clinical documentation.

Key responsibilities

• Provide weekly clinical support to assigned clients, including assessments, treatment planning, and progress monitoring.
• Coordinate care with therapists, psychiatric providers, case managers, and community resources.
• Track referrals for services such as housing, substance use treatment, primary care, and other community supports.
• Maintain accurate documentation in the electronic medical record and ensure compliance with agency, payer, and regulatory standards.
• Serve as Treatment Team Lead, offering coaching, feedback, and assistance to team members.
• Participate in supervision, audits, trainings, and quality‑improvement activities.

Required profile

• Active Maryland license or certification (e.g., LCPC, LCSW, LCMFT, LCADC, APRN‑PMH, Licensed Psychologist, etc.).
• Experience in outpatient or community mental health settings preferred.
• Knowledge of trauma‑informed, person‑centered, and recovery‑oriented care.
• Strong leadership, communication, organization, and time‑management abilities.
• Reliable transportation for onsite or community‑based service needs.
• Compassionate, collaborative, and culturally responsive approach.

Required skills

• Electronic medical records (EMR) proficiency.
• Basic computer applications.
