Transition Team Registered Nurse – Private Duty

About the role

Join BAYADA's Transition Team as a full‑time Registered Nurse providing complex, private‑duty care to infants, children, and adults in Chester and Montgomery County. You will bridge hospital discharge to ongoing home care, delivering high‑quality care and training the permanent care team.

Key responsibilities

• Coordinate and execute the transition of care from hospital to home for patients with rare or complex conditions.
• Provide direct nursing care, including tracheostomy and ventilator management, in a home setting.
• Educate and train the ongoing BAYADA care team on patient-specific protocols.
• Travel throughout Chester, Delaware, and parts of Montgomery County to meet patient needs.

Required profile

• Current Registered Nurse license.
• Experience in NICU, PICU, ICU, or home‑care trach/vent settings.
• Current CPR certification.
• Ability to maintain a consistent schedule and work around pets.

Required skills

• High‑tech adult and pediatric simulation competency (passing score required).
• Tracheostomy and ventilator management expertise.
• NICU/PICU/ICU clinical experience.
• CPR certification.
