Transition Team Licensed Practical Nurse – Private Duty

About the role

Join BAYADA's Transition Team as a Licensed Practical Nurse (LPN) providing private‑duty care for infants, children, and adults with complex medical needs in Chester, Delaware, and Montgomery Counties. This full‑time position supports the hand‑off from hospital to home care and offers extensive training and mentorship.

Key responsibilities

• Deliver high‑quality, complex care to clients in their homes, including tracheostomy and ventilator management.
• Facilitate the transition of care from hospital teams to ongoing home‑care services, training the permanent BAYADA care team.
• Travel throughout Chester, Delaware, and parts of Montgomery County to meet client needs.

Required profile

• Current Licensed Practical Nurse (LPN) license.
• Experience in NICU, PICU, ICU, or home‑care trach/vent settings.
• Ability to maintain a consistent schedule and commit to ongoing training duties.

Required skills

• NICU, PICU, or ICU clinical experience.
• Tracheostomy and ventilator care expertise.
• Proficiency with high‑tech adult and pediatric simulation assessments.
• Current CPR certification.
