Telehealth Transitional Care Nurse Practitioner

About the role

Join urrly’s remote, member‑facing clinical team to provide post‑discharge care through telehealth. You will conduct Transitional Care Management (TCM) visits within 14 days of discharge, perform comprehensive assessments, reconcile medications, and help reduce avoidable readmissions.

Key responsibilities

• Conduct virtual TCM visits, complete clinical assessments, and perform medication reconciliation for recently discharged members.
• Identify red‑flag symptoms, escalate urgent clinical needs, and coordinate care with primary care physicians, specialists, and discharge planning teams.
• Document encounters accurately and submit charts on the same day to support coding, quality reporting, and claims submission.

Required profile

• Active, unrestricted Nurse Practitioner licensure in at least three states (e.g., OH, CA, NY, IN, KY, PA, LA, MI, TN, MO).
• National board certification (AANP or ANCC) as an FNP, AGNP, or equivalent adult/geriatric certification.
• Minimum two years of clinical experience and comfort delivering care via video visits.

Required skills

• Proficiency with EMR documentation platforms.
• Experience using video‑visit software and point‑of‑care technology on desktop or mobile devices.
• Ability to maintain a private, HIPAA‑compliant workspace with reliable high‑speed internet.
