Supervisor Care Management - Social Worker

About the role

AdventHealth in Palm Coast, Florida is looking for a full-time day-shift Supervisor Care Management - Social Worker. The role supervises daily operations of discharge planning and social services in the inpatient, observation and emergency departments, acting as the main point of contact for Care Management barriers and concerns on the inpatient unit.

Key responsibilities

• Implement corporate Care Management strategies with facility management.
• Take part in multidisciplinary rounds and give discharge planning and care progression expertise to the interdisciplinary team.
• Use reports and dashboards to monitor the accuracy of care managers' documentation and compliance with regulations.
• Facilitate care conferences for complex transitions and placements, and join hospital and medical staff meetings to review high-dollar, long-stay or difficult-placement patients.
• Report quality and risk issues promptly, and handle patient, staff, physician and department complaints with follow-up and escalation.
• Take part in hiring, onboarding, training, evaluation and mentoring of staff.
