State Traveler Nurse Practitioner (Illinois)

About the role

Advantmed, a healthcare solutions company focused on risk adjustment, quality improvement and patient engagement, is looking for a Nurse Practitioner to join its full-time provider network and support members across Southern Michigan. The State Traveler performs in-home wellness visits and risk adjustment assessments for Medicare and other eligible populations, contributing to accurate clinical documentation, identification of care gaps and value-based care.

Key responsibilities

• Travel to members' homes and conduct comprehensive health evaluations, including annual wellness visits and chronic condition assessments.
• Perform physical examinations, review of systems, vital signs, medication review and reconciliation, and preventive screenings such as hemoglobin A1C, spirometry, kidney health evaluation, diabetic retinal exam, fecal immunochemical test, bone mineral density screening and more.
• Educate patients on disease prevention, medication adherence and chronic condition management, obtain consent, and hold goals-of-care discussions when appropriate.
• Document findings accurately, identify conditions for risk adjustment, close quality care gaps and collaborate with interdisciplinary teams.

Required profile

• Active, unrestricted Nurse Practitioner license in Illinois.
• National board certification in the NP specialty.
• At least 3 years of direct patient care experience in primary care, family practice, internal medicine, geriatrics, urgent care or similar settings.
• Valid driver's license and reliable transportation; ability to travel extensively and to enter and work in patient homes.
• Preferred: experience with Medicare populations, prior in-home assessment or risk adjustment visits, EMR proficiency, bilingual skills, familiarity with value-based care and quality gap closure.
