Home Health Speech-Language Pathologist (Part-Time)

About the role

AdvisaCare is hiring Speech-Language Pathologists (SLPs) for its home health therapy teams in St. Clair Shores, Eastpointe, Grosse Pointe, Monroe and Belleville. The home health census is growing, and the agency offers full-time, part-time and PRN positions. As an SLP you provide personalized, one-on-one care to help patients improve communication, cognition, swallowing, safety and quality of life in their own homes.

Key responsibilities

• Complete speech-language evaluations of communication, cognition, voice and swallowing needs.
• Develop individualized plans of care and measurable treatment goals.
• Provide evidence-based interventions for speech, language, cognitive-communication and swallowing disorders, including dysphagia, aphasia, dysarthria and cognitive impairment.
• Reassess patient progress and modify treatment plans as clinically appropriate.
• Provide skilled speech therapy in line with physician orders and CMS Conditions of Participation.
• Educate patients and caregivers on communication strategies, swallowing precautions, aspiration prevention, cognitive techniques, adaptive tools and Home Exercise Programs.
• Maintain accurate documentation per CMS, payer and agency requirements.
• Collaborate with physicians, caregivers, nursing and the interdisciplinary team.

Required profile

• Degree from an accredited Speech-Language Pathology program.
• Current Michigan Speech-Language Pathologist license in good standing.
• Certificate of Clinical Competence (CCC-SLP) preferred.
• Home health experience preferred.
• Reliable transportation, valid driver's license and current auto insurance.

Required skills

• Familiarity with Medicare/CMS documentation preferred.
• Strong clinical judgment, critical thinking and problem solving.
• Excellent communication and patient education skills.
• Ability to work independently while staying connected to the care team.
